Employ clear, straightforward language, using large/easy to read font sizes in communications where appropriate, utilizing visual aids like Makaton signs or symbols to guarantee comprehension.

Designate a single point of contact (lead practitioner) to manage information and ensure consistent messaging from all professionals involved in the individual’s care.
Health Action Plans & Hospital Passports:
These instruments document health information, support requirements, and the individual’s preferences, aiding in the continuity of care and facilitating easy access to information.
Professional & Emotional Support Training and Expertise:
Ensure that staff receive sufficient training in learning disabilities and person-centred thinking to enhance their ability to support the individual and family.
